on some tough challenges.The schedule for this position is Monday -
Friday: Three days/week are standard business hours. Two days/week
are 11am-8pm. Schedules are in the local time zone.Primary
Responsibilities:
- Making outbound calls to assess members' current health
status
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self-management
- Interacting with Medical Directors on challenging cases
- Coordinating care for members
- Making referrals to outside sources
- Coordinating services as needed (home health, DME, etc.)
- Educating members on disease processes
- Encouraging members to make healthy lifestyle changes
- Documenting and tracking findings
- Making 'welcome home' calls to ensure that discharged member
receive the necessary services and resourcesThis is high volume,
customer service environment. You'll need to be efficient,
productive, and thorough dealing with our members over the phone.
Solid computer and software navigation skills are critical. You
should also be exceptionally patient-focused and adaptable to
changes.This position will require active and unrestricted Nursing
licensure in multiple US States. Selected candidate must be willing
and able to obtain and maintain multiple state licensure.

you may be interested in.Required Qualifications:
- Active, unrestricted Compact RN License in your state of
residence
- Willing and able to obtain additional RN licenses as
necessary
- 3+ years of RN experience in a hospital setting, acute care,
direct care, or as a telephonic Case Manager for an insurance
company
- Basic Microsoft Word proficiency with the ability to navigate a
Windows environment
- Full COVID-19 vaccination is an essential requirement of this
role. Candidates located in states that mandate COVID-19 booster
doses must also comply with those state requirements. UnitedHealth

well as all client requirements and will obtain necessary proof of
vaccination, and boosters when applicable, prior to employment to
ensure compliancePreferred Qualifications:
- Bachelor's Degree
- Active and unrestricted RN license in Minnesota
- Certified Case Manager (CCM)
- Managed Care experience
- Case Management experience
- Call Center experience
- Experience / exposure with discharge planning
- Experience with utilization review, concurrent review and / or

or Nevada Residents Only: The salary range for Colorado residents
is $54,400 to $97,000. The salary range for Connecticut / Nevada
residents is $60,000 to $106,700. Pay is based on several factors
including but not limited to education, work experience,
certifications, etc. In addition to your salary, UnitedHealth Group
