(LPN) provides nursing care to assigned patients under the direct
supervision of the registered nurse (RN) or other assigned
supervisor and accepts delegation from the RN/assigned supervisor
in meeting the needs of the patient/family. The LPN collects data
through observation and communicates information to assist the RN
and/or provider in patient assessment and care planning. The LPN
participates as a member of the health care team and accepts
delegation from the RN/assigned supervisor and provider in meeting
needs of the patient/family. The LPN delivers care in accordance
with the patient care plan, policies and procedures of the
organization and principles of relationship-based care. The LPN
possesses excellent communication skills; is skillful in mentoring
and instructing; and may participate on committees or projects,

Position Highlights: Limited weekend rotation: only 4–6 Saturdays
per year Work with a collaborative, multi-disciplinary team of
physicians, advanced practice providers, and nursing colleagues

support across a variety of family medicine services Provide

Qualifications: /n /n - Graduate of a school of practical nursing
or passed LPN Boards after a defined period in a professional
nursing program (e.g., some states allow RN students to take the
LPN board exam after completing one to two years in the RN
program). /n - If graduation did not occur within the last two
years, one year of LPN experience working in an applicable setting
is required. /n - One year LPN experience within the last five
years preferred. /n - Excellent communication skills (verbal and
written). /n - Experience working in a team environment. /n -
Computer skills required, prior experience with electronic medical
record systems preferred. /n - Ability to work daytime hours,
flexibility may be required to meet staffing needs. /n - Ability to
adapt to unpredictable situations within the work setting. /n /n
License and Certifications: /n /n - Current LPN license by
applicable state requirements. /n - BLS (Basic Life Support) must
be valid for 90 days post start date – acceptable BLS certificates
are American Heart Association: “BLS Provider” or American Red
Cross: “Basic Life Support” or “CPR/AED for Professional Rescuers
and Health Care Providers” /n - Additional specialty
certification/training as required by the work area. Positions that
are not on campus may not require current Basic Life Support (BLS)
competency as determined by the work area. /n Exemption Status
