Job Description Job Title: Family / Internal / Geriatric Medicine Physician Location: Forest Lake, MN Shift: Full-time & Part-time position Summary: We are looking for a Physician to serve patients in Long-Term Care (LTC) and Transitional Care Units (TCUs). Responsibilities: - Conduct comprehensive patient assessments and ongoing medical evaluations. - Create and manage personalized treatment plans based on patient health needs. - Diagnose and treat acute and chronic conditions, providing timely medical interventions. - Prescribe medications, order diagnostic tests, and modify treatments as needed. - Work closely with nurses, therapists, and social workers to coordinate comprehensive care. Requirements: - MD or DO degree from an accredited medical school. - Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred. - Valid medical license to practice in Minnesota. - Previous experience in long-term care, transitional care, or geriatrics is a plus. - Excellent problem-solving, communication, and teamwork skills.
